Promoting Early Learning in the Medical Home

Promoting child health and development is a fundamental part of pediatric primary care, commonly through:

  •  Well Child Care: regular check ups to assess health and development, and provide anticipatory guidance to parents about supporting their child’s health and development
  •  Reach Out and Read: implementation of this proven program that promotes early literacy by giving new books to children and advice to parents about the importance of reading aloud within the Medical Home.  (see Reach Out and Read Washington State)
  •  Developmental Screening: using standardized tools to assess child development, and provide referral for further assessment as needed

In addition, the medical home can make referrals to community services which support parents, and promote early learning, such as

  •  Child Care Resource and Referral Programs
  •  Preschool Programs, such as Head Start and ECEAP
  •  Home Visiting Programs

Stay tuned as we begin to feature projects and resources in these areas.
